If you would like to transfer care to our office please fill out our Records Request form so we can obtain records from your child's previous office.

If you need records transferred from our office to another or need information from your child's chart, please fill out our Records Release form. 

If you need to give consent to someone other than a parent/guardian, please fill out our Consent to Care form. 

After forms are completed, please return them to our office either by mail, fax, email, or you can drop them off during normal business hours. Please keep in mind we do require one form filled out per child.
